Spinal Stenosis is the narrowing of the spaces within your spine, which can put pressure on the spinal cord and the nerves that travel through the spine. This condition is most common in the lower back (lumbar spine) and neck (cervical spine) and can lead to symptoms ranging from chronic pain and stiffness to debilitating pain, numbness, or cramping in the legs, a symptom often referred to as Lumbar Radiculopathy.
At Dosher Physical Therapy, our specialized Stenosis Management program focuses on non-surgical, evidence-based strategies to create space in the spine, reduce nerve compression, and maximize your comfort and walking tolerance. We empower patients in the Upstate area to manage their symptoms and return to a more active, pain-free life.
What is Spinal Stenosis Management?
Physical therapy is highly effective as the primary non-surgical intervention for spinal stenosis. The goal of management is to utilize specific exercises and postural adjustments that temporarily open the narrowed spinal canal, reducing pressure on the affected nerves. Our program focuses on:
- Flexion Bias: Identifying and training positions that provide relief—typically those that involve a gentle forward bend (flexion), such as leaning on a shopping cart or bike. This contrasts with extension movements, which typically worsen symptoms.
- Core and Stability Training: Building strong, resilient Core Strength and Stability Programs to help the patient maintain the pain-relieving flexed posture effortlessly during daily activities and walking.
- Endurance and Walking Tolerance: Working to increase the distance and time you can walk without experiencing leg pain or cramping (neurogenic claudication).
- Addressing Co-occurring Conditions: Treating the underlying factors that contribute to nerve irritation, such as associated Degenerative Disc Disease Management or stiffness in the surrounding joints.
Who Can Benefit from Stenosis Management?
If you experience pain or functional limitations that are relieved by sitting or bending forward, you are a strong candidate for our specialized therapy. This includes individuals with:
- Neurogenic Claudication: Leg pain, cramping, or heaviness that is brought on by standing or walking and is relieved quickly by sitting or bending forward.
- Chronic Low Back or Neck Pain: Persistent stiffness and ache related to the narrowing of the spinal canal.
- Radiculopathy Symptoms: Numbness, tingling, or weakness in the legs or arms caused by nerve compression.
- Difficulty Walking: Reduction in the distance you can walk before needing to sit and rest.
- Diagnosis of Osteoarthritis: Stenosis is often a consequence of age-related arthritis and bony overgrowth in the spine.
Key Treatment Techniques Used
A successful stenosis management plan relies on movement and positioning that reduces compression:
Technique | Focus Area | Benefit for Stenosis |
Flexion-Based Exercises | Specific movements and postures that favor a gentle forward-bent position. | Temporarily opens the spinal canal, reducing compression and leg symptoms. |
Manual Therapy | Gentle traction and specific joint mobilizations (avoiding extension). | Helps decompress the spine and improve the mobility of adjacent joints. |
Core Strength and Stability | Training abdominal and pelvic muscles to maintain a protective spinal posture. | Provides the muscular support needed to perform daily tasks with less pain. |
Stretching Programs | Stretching tight hip flexors and back extensor muscles. | Reduces muscular pull that can promote a painful, extended posture. |
Pain Management | Using modalities and education to control chronic back and leg discomfort. | Empowers the patient to reduce pain sensitivity and increase daily activity levels. |

Additional Resources and Services
Stenosis management is often integrated with care for related spinal issues:
- Lumbar Radiculopathy Management: Specific focus on the nerve-related symptoms in the legs.
- Spondylolisthesis and Spondylosis Management: Addressing instability or arthritis that frequently co-exists with stenosis.
- Postural Taping and Support: Providing external cues to help maintain the pain-relieving flexed or neutral posture.
