On your first day it is best to arrive a few minutes early to complete our intake paperwork. Then you will have an evaluation and treatment by one of our Physical Therapist. Please wear athletic or lose fitting comfortable clothing to allow ease of treatment and evaluation. Each session typically lasts an hour. Please download our New Patient Packet and complete prior to your first day to speed the initial paperwork process.

Please note, in order for treatment to be covered by Medicare, Medicaid or Advantage plans, patients must be under the care of a physician who must sign our plan of care or provide a referral for the treatment to be covered.

Yes. We’re happy to serve patients who elect to self-pay as well as patients who plan to utilize insurance coverage, Medicare, Medicaid or Advantage plans. Our patient care team can assist with billing and insurance questions.
