When you experience sharp joint pain or stiffness when performing a specific movement (like lifting your arm, turning your neck, or squatting), it often means the joint’s alignment or “tracking” is slightly off. Mulligan’s Techniques, also known as Mobilization with Movement (MWM), are an advanced form of Manual Therapy designed to instantly correct this positional fault, allowing you to perform the painful movement without discomfort.
At Dosher Physical Therapy, our skilled clinicians use MWM to provide immediate, often dramatic, relief and restored function. It’s a powerful tool that transforms painful, restricted movement into pain-free motion, accelerating your return to daily activities and sports.
What are Mulligan’s Techniques (MWM)?
Mulligan’s Techniques are a patient-centered, hands-on treatment approach where the therapist applies a sustained, accessory mobilization to a joint while the patient actively moves.
The core principle is simple: if a specific joint movement is painful or limited, the therapist gently applies a directional glide or translation to the joint (the mobilization) until the movement becomes pain-free. The patient then actively repeats the previously painful movement multiple times while the glide is maintained.
- Pain-Free Principle: If the technique is correct, the movement should be completely pain-free. If pain remains, the therapist adjusts the glide until the Pillars of MWM are met.
- Restoring Positional Faults: MWM is effective because it temporarily corrects a minor positional fault (a subtle misalignment) that causes pinching or friction during movement.
- Immediate Results: Patients often experience an immediate increase in range of motion and a reduction in pain that is then reinforced through repeated pain-free practice.
Who Can Benefit from Mulligan’s Techniques?
Mulligan’s Techniques are highly effective for peripheral joints (limbs) and certain spinal conditions where pain is clearly linked to specific movements. Our therapists use MWM as part of the treatment for:
- Shoulder Pain: Restoring pain-free overhead reaching for conditions like Impingement Syndrome Rehabilitation or post-Rotator Cuff Repair Rehabilitation stiffness.
- Knee Pain & Tracking Issues (Patellofemoral): Correcting the glide of the kneecap to eliminate pain during squatting or stair climbing.
- Ankle Sprains: Improving the function and stability of the stiff ankle joint after injury (Foot and Ankle Rehabilitation).
- Elbow Pain: Used to relieve pain associated with Tennis and Golfer’s Elbow when gripping or lifting is painful.
- Neck Pain and Strain Rehabilitation: Immediately improving range of motion when turning the head is painful or restricted.
- Jaw and TMJ Treatments: Improving the tracking and opening mechanics of the temporomandibular joint.
Key Treatment Techniques Used
MWM is a dynamic, active approach that immediately transitions into self-management:
Technique | Focus Area | Goal |
Mobilization with Movement (MWM) | Therapist glides the joint while the patient actively performs the motion. | Achieve instant pain relief and restore full range of motion. |
Sustained Natural Apophyseal Glides (SNAGs) | Specific MWMs applied to the spinal joints (neck and back). | Improve spinal mobility that contributes to Neck Pain and Lumbago. |
Active Self-Correction | Patient learns to replicate the successful glide with a belt or towel. | Empowers the patient to treat themselves at home and maintain the correction. |
Reinforcement | Strengthening exercises are immediately performed in the newly acquired pain-free range. | Locks in the functional gains and prevents the positional fault from recurring. |

Additional Resources and Services
MWM is often integrated with other advanced services for a comprehensive approach:
- Joint Manipulations and Mobilizations: Traditional mobilization techniques are often used alongside MWM.
- Proprioception Exercises: Training the body’s awareness to maintain the corrected joint position during dynamic activity.
- Postural Taping and Support: Applying tape to cue and maintain the alignment achieved through MWM.
