A Functional Capacity Evaluation (FCE)is a comprehensive, objective testing method used to determine an individual’s current functional abilities and limitations in relation to their work demands and daily life activities. At Dosher Physical Therapy, our highly trained therapists conduct detailed FCEs to provide accurate, reliable data for patients, physicians, case managers, employers, and legal professionals in the upstate area.
Whether you are seeking to determine a safe return-to-work status, evaluate disability, or measure the effectiveness of therapy like Work Conditioning, a professionally administered FCE is the gold standard for objective functional measurement.
What is a Functional Capacity Evaluation (FCE)?
An FCE is typically a one- or two-day evaluation (depending on complexity) that measures a patient’s maximum safe capacity across a range of physical demands. The evaluation compares the patient’s performance against the requirements of their specific job (or a general set of work demands).
The FCE measures key parameters including:
- Material Handling: Lifting, carrying, pushing, and pulling (both maximum weight and frequency).
- Postural Tolerance: Bending, stooping, crouching, kneeling, and crawling.
- Non-Material Handling: Hand dexterity, coordination, and reaching.
- Endurance: Sustained activities over time.
- Consistency and Reliability: Techniques are used to determine the consistency of effort given by the patient.
The final report provides clear, reliable documentation detailing the patient’s safe work capability, classified by physical demand levels (e.g., Sedentary, Light, Medium, Heavy, Very Heavy).
Who Needs an FCE?
FCEs are necessary in several key scenarios related to occupational health and injury management:
- Return-to-Work Decisions: To determine if an employee can safely return to their previous job, a modified job, or requires Work Conditioning to bridge the gap.
- Disability Determination: To provide objective evidence for short-term or long-term disability claims.
- Worker’s Compensation Cases: To establish the existence of permanent physical impairments and maximum medical improvement (MMI).
- Legal or Settlement Cases: Providing objective physical evidence for personal injury claims.
- Before Work Conditioning: Serving as a baseline measure before beginning intensive work-focused rehabilitation.
The Dosher FCE Difference
Our FCEs are conducted by experienced therapists and adhere to strict, peer-reviewed protocols to ensure validity and defensibility:
- Job-Specific Focus: We tailor the evaluation to the actual demands of the patient’s job description whenever possible.
- Safety First: The therapist continuously monitors the patient’s vital signs and movement mechanics to ensure all tests are performed safely and within the patient’s capabilities.
- Objective Reporting: Our reports are detailed, clear, and easy to understand for all involved parties (physicians, case managers, and lawyers).
- Reliability Assessment: We incorporate tests to assess the effort and reliability of the patient’s performance, ensuring the data is medically valid.

Additional Resources and Services
FCEs are often the starting point for work-related recovery and rehabilitation:
- Work Conditioning: Intensive, job-related training following an FCE to build stamina and strength for returning to work.
- Orthopedic Post-Operative Rehabilitation: Measuring function after major surgery (like Total Hip or Knee Replacement) before returning to manual labor.
- Pain Management (Chronic and Acute): Identifying how pain levels impact functional ability and using the FCE as a tool to measure improvement.
