Proprioception is your body’s “sixth sense”—it’s the unconscious awareness of where your body parts are in space without having to look at them. This sense is crucial for automatic balance, quick reaction time, and stable joint function. When you suffer an injury, especially a ligament tear or sprain (like a rolled ankle or ACL Reconstruction), this sense is temporarily disrupted, leaving the joint feeling unstable and prone to re-injury.
At Dosher Physical Therapy, Proprioception Exercises are a core component of our rehabilitation programs. We specialize in re-training the nervous system to improve joint awareness and reaction time, providing a solid foundation of stability for all functional activities.
What are Proprioception Exercises?
Proprioception training involves exercises that challenge your balance and coordination, forcing the muscles and joints to communicate rapidly with the brain. The goal is not just to build physical strength (General Strengthening Exercises) but to build neuromuscular control—the brain’s ability to tell the right muscle to fire at the right time.
Our proprioception programs are tailored to the injured joint but often include:
- Balance Training: Progressing from stable surfaces (the floor) to unstable surfaces (foam, rocker boards, Bosu balls) to challenge the body’s reaction to uneven surfaces.
- Kinesthesia Training: Re-training the ability to detect joint movement and position, often performed with the eyes closed to remove the visual sense and focus solely on joint feedback.
- Perturbation Training: Applying unexpected, gentle disturbances to the body (such as pushes or pulls) to train rapid balance-recovery reactions, which are vital for Fall Prevention Programs and sports.
Who Can Benefit from Proprioception Training?
Proprioception training is crucial for safety, stability, and high-level function. It is essential for anyone who:
- Has Joint Instability: Particularly after ankle sprains, where chronic instability is common, or knee injuries (ACL Reconstruction Rehabilitation).
- Is Recovering from Surgery: Post-operative joints (Orthopedic Post-Operative Rehabilitation) have significantly reduced proprioception and require specific retraining before returning to activity.
- Needs Fall Prevention: Proprioception is a core component of Balance and Fall Prevention (including Parkinson’s Disease Programs), as quick reaction time prevents stumbles from becoming falls.
- Is an Athlete: To improve agility, cutting ability, and reaction time for Athletic Performance & Return-to-Sport Training, minimizing the risk of a non-contact injury.
- Deals with Neurological Issues: Essential for patients in Neurological Rehabilitation to improve motor control and spatial awareness after a stroke or Spinal Cord Injury.
Key Proprioception Drills
Proprioception exercises are always performed under the careful supervision of a therapist to ensure safety and proper challenge level:
Drill Type | Focus Joint | Goal |
Single-Leg Stance | Foot and Ankle Rehabilitation, Knee. | Improves static balance and the endurance of small foot and ankle stabilizers. |
Rocker/Wobble Board | Ankle, Knee. | Trains dynamic stability and quick reaction of muscles to surface shifts. |
Star Excursion Balance Test (SEBT) | Hip, Knee, Ankle. | Challenges multi-directional stability and functional reach limits. |
Perturbations | Trunk, Core, and Legs. | Trains the core and extremities to react and stabilize the body against unexpected forces. |
Eyes-Closed Activities | All Joints. | Removes visual input to force reliance on the somatosensory (proprioceptive) system. |

Additional Resources and Services
Proprioception is a skill integrated across many therapeutic services:
- Core Strength and Stability Programs: A stable core is the foundation for proprioceptive control in the limbs.
- Custom Shoe Orthotic Evaluations and Fittings: Provides a stable, consistent base of support for the foot, enhancing overall body proprioception.
- Postural Taping and Support: Taping can provide enhanced sensory feedback to the skin, which assists proprioception.
