IASTM: Soft Tissue Mobilization

Instrument-Assisted Soft Tissue Mobilization (IASTM) is an advanced form of manual therapy used by our skilled physical therapists to address muscle, tendon, ligament, and fascial restrictions. This technique utilizes specialized stainless steel or plastic tools to precisely detect and effectively treat areas exhibiting scar tissue, chronic inflammation, or fascial tightness.

At Dosher Physical Therapy, IASTM is a powerful tool integrated into treatment plans to break down abnormal tissue adhesions, stimulate a localized inflammatory response for healing, and quickly restore full range of motion and function to the injured area.

What is Instrument-Assisted Soft Tissue Mobilization (IASTM)?

IASTM is based on the principles of soft tissue mobilization, but the instruments allow the therapist to apply deeper, more focused pressure than is possible with hands alone. The stainless steel edges of the tools resonate with vibrations and inconsistencies in the tissue, allowing the therapist to “feel” restrictions and treat them with high precision.

The therapeutic effects of IASTM include:

  • Breaking Up Scar Tissue: Mobilizing connective tissue adhesions that often form after injury or surgery, which can restrict movement and cause chronic pain.
  • Initiating Healing: Creating microtrauma in the tissue, which stimulates an increased blood flow and cellular activity necessary for repair (Soft Tissue Mobilizations).
  • Reducing Fascial Restriction: Releasing the tight connective tissue (fascia) that envelops muscles, immediately improving flexibility and reducing tension.
  • Pain Reduction: Modulating the nervous system’s pain signals, leading to rapid symptomatic relief.

Who Can Benefit from IASTM?

IASTM is highly effective for chronic conditions and repetitive strain injuries where scar tissue and persistent tightness are a factor. It is often used as a complementary technique for:

  • Tendinopathies: Conditions like Tennis Elbow and Golfer’s Elbow (Elbow Rehabilitation), Achilles Tendinopathy, or Rotator Cuff tendinosis.
  • Plantar Fasciitis Management: Releasing the tight and fibrotic tissue in the arch of the foot.
  • IT Band Condition Treatment: Mobilizing the lateral thigh fascia to reduce friction and pain.
  • Neck and Back Pain: Treating chronic tension and restrictions in the muscles of the spine.
  • Post-Operative Rehabilitation: Addressing scar tissue around surgical sites (e.g., after Rotator Cuff Repair or ACL Reconstruction Rehabilitation).
  • Chronic Joint Stiffness: Used to release surrounding soft tissues that are limiting motion in joints like the hip or shoulder.

Key Treatment Techniques Used

While IASTM is the core technique, it is always integrated with movement-based therapy:

Technique

Focus Area

Goal

IASTM

Application of specialized tools to glide, sweep, and mobilize restricted tissue.

Directly targets and breaks down collagen cross-links (scar tissue).

Stretching Programs

Stretching immediately following IASTM treatment.

Locks in the range of motion gains achieved by the mobilization.

Eccentric Strengthening

Loading the treated tendons/muscles while lengthening.

Rebuilds the strength and elasticity of the tissue with proper load.

Manual Therapy

Traditional hands-on techniques often used before or after IASTM.

Complements the instrument work by addressing joint mobility.

Locations

Instrument-Assisted Soft Tissue Mobilization (IASTM) is performed by certified therapists at all five of our Dosher Physical Therapy clinics:

  • Anderson, SC
  • Easley, SC
  • Pickens, SC
  • Powdersville (Piedmont), SC
  • Clayton, GA

Additional Resources and Services

IASTM is frequently used in conjunction with other manual and movement therapies for best results:

  • Cupping Therapy: A complementary technique that uses negative pressure to lift and decompress soft tissue.
  • Dry Needling: Treating deep, focal trigger points that IASTM may not reach.
  • General Strengthening Exercises: Building the muscular support necessary for long-term protection of the mobilized tissue.
